Instructions to Authorize a 学生记录 Release
Under the requirements of the Family Educational Rights & 隐私法(FERPA), Loyola will not discuss or disclose any elements of your 教育 record except for “directory information” without your explicit consent. 提供此同意, you must complete the online 学生记录 Release form in 罗拉自助服务 for each individual that you wish to authorize Loyola to discuss your 教育 records with. 没有这个授权, Loyola will not respond to requests for information about your 教育 record from an unauthorized individual.
To create a new records 释放 authorization or modify an existing one, follow these steps:
1) In 罗拉自助服务, select “学生记录 Release” under “User Options”
2) If you have any previously authorized individuals with student records 释放s, 你可以在这里查看. 其他wise, select “Add Person / Relationship” to create a new 释放.
You will be asked to create a PIN for the named individual. Loyola may ask this person to provide the PIN as verification when they contact the university about your student record. You also will be asked to specify the start and end date of the period in which you authorize the 释放 of your 教育 information to the named individual. You can leave either or both of these dates blank to provide 释放 for an indeterminate time, but you may also specify a date range and Loyola will not 释放 information outside of those dates.
3) You can select either to authorize complete access to your 教育 record for the named individuals or to select from the choices provided. 如果你选择“完全访问”, Loyola will still refrain from sharing certain data such as HIPAA-protected medical records or other sensitive data as required by other regulations. 这是一个 教育 释放.
If you choose “Select Access”, you may indicate which items you do or do not authorize:
- Academic Information includes but is not limited to grades, 日程安排, 站, 进展程度, 转移工作, 学术违规, 停职或留校察看, and any registration restrictions.
- 计费 & Student Accounts includes but is not limited to all items and detail from your billing statement, including any tuition and fees charged, 食宿费用, any transmitted financial aid (including Federal and State aid), 退款或罚款, 你的活期存款是多少?.
- Disability Accommodations includes but is not limited to any services rendered or approved by the Office of 访问教育 or other offices in response to an approved request from you.
- 金融援助 & 奖学金 includes but is not limited to any financial aid awards you are offered or eligible for, any Federal aid eligibility or calculations, 您提交的FAFSA信息, any records relevant to satisfactory academic progress status, and ongoing financial aid eligibility.
- 学生事务 & Conduct includes but is not limited to participation in any student organizations such as intercollegiate athletics or Greek life (including eligibility and current status), any disciplinary records or other student conduct information, and any residential information other than billing.
4) After you select the desired access level for the 释放, scroll down and provide consent. 如果进程成功, you will be able to verify the status of the records 释放 on your page. You can use this same form at any time to add additional authorized individuals. 注意: You are not yet able to modify record 释放s on your own and must contact the Office of 学生记录 to modify or revoke an authorized 释放. 在不久的将来, we anticipate that the ability for students to modify authorized 释放s will become available.
